WAIVER AND RELEASE OF LIABILITY

The undersigned participant, in consideration of participating in the veterinary clinic

entitled “Simplifying Dental Extractions in the Cat/Dog” which consists of a veterinary dental extraction wet lab, hereby waives and releases, indemnifies, holds harmless, and forever discharges Veterinary Dentistry and Oral Surgery of Rhode Island Inc. (hereinafter VDOSRI), and its officers, agents, associates, and affiliates of and from any liabilities, of any kind and nature, whether known or unknown, in law or in equity, that I ever had or may have, arising from, or in any way related to my participation in the aforementioned feline/canine dental extraction clinic.

I acknowledge that the extraction clinic involves the use of high-speed dental hand

pieces with cutting burs, tools, and other sharp-edged instruments and objects, that are

capable of causing physical injury, serious physical injury and/or death. On behalf of

myself, my heirs, and assigns, I hereby waive any and all claims for damages resulting from any injuries or death that I may sustain during my participation in the feline dental

extraction clinic that may have been pursued against VDOSRI, and its officers, agents,

associates, and affiliates. I assume the risk that injury may occur as a result of my

participation in the feline/canine dental extraction clinic and I specifically waive any and all claims for damages or liability that may arise therefrom. 

This waiver is a contract between myself and VDOSRI, the consideration being my allowed participation in the feline/canine dental extraction clinic, and this waiver and release of liability is intended to give the broadest legal effect permitted by law.

I have read, understand, and fully agree to the terms of this waiver and release of

liability. I understand and confirm that by signing this waiver and release, I have given up considerable future legal rights. I have signed this waiver and release voluntarily, while under no duress or threat of duress. By my signature I confirm my intent to execute a full and complete waiver and release of liability to the fullest extent permitted by law. I am 18 years of age or older and I am mentally competent to enter into this waiver and release of liability.

Receipt of the voluntary waiver and release is hereby acknowledged by VDOSRI.
